Some risks of HRT may include the following: Testosterone to estrogen conversion, for that we advise an estrogen blocker (usually anastrozole). Testosterone has the potential to decrease your body’s own production of testosterone & sperm production, sometimes causing testicular shrinkage (clomiphene, gonadorelin or kisspeptin may help with this). Testosterone may cause an increase in red blood cell production in some patients, to help this, we recommend you donate blood several times per year (if you are unable to donate, you can have a therapeutic phlebotomy performed - alert your coordinator if you need help with this). There is a possibility that HRT has the potential to decrease sperm count and may affect fertility. There have not been enough research/studies of pre-HRT/post-HRT sperm counts to be definitive. Should the time come for you to want to conceive, alert your patient coordinator, so they may adjust your protocol to boost sperm production.
